Case Management

Case Management

Unresolved secondary issues can often contribute to relapse for the recovering woman. Therefore, case managers at Hope House give special attention to each individual client and her needs. Treatment Plans and Reviews are completed by each resident with her case manager on a regular basis, outling specific problem issues to be resolved and setting goals in order to obtain the desired outcome. Client progress is assessed by case managers weekly, with full-staff assessments bi-weekly. Depending on the length of individual residency, the woman can accomplish a variety of goals which may include employment, education, resolution of legal issues, counseling, as well as family reunification. Hope House is proud to announce that many of the women who complete the Hope House Program maintain long-term recovery from chemical dependency.
